Care Management

Care coordination and disease coaching are part of your health benefits and are provided to you at no cost. MHS pays for these services. We provide services for many conditions, such as asthma, diabetes, COPD, high-risk pregnancy and many more. We also offer a Children with Special Needs program for children with developmental or chronic health conditions.

If you have, or are at risk for having, one of the following health conditions listed, please call so that we can tell you more about and offer to enroll you in a program. If you would like to find out more, or to join a program, please contact us.

My Health Pays® Rewards for Disease Management Coaching

You can earn up to $85 per year in My Health Pays® rewards by participating in disease coaching for certain conditions. You can use reward dollars to pay your monthly POWER Account contribution. Use your My Health Pays® rewards to help pay for everyday items at Walmart*, utilities, transportation, telecommunications (cell phone bill), childcare services, education and rent.

Conditions that qualify for rewards: 

  • Asthma
    Asthma cannot be cured, but most people with asthma can control their symptoms and prevent asthma attacks by avoiding asthma triggers and correctly using prescribed medicine, such as inhaled corticosteroids.
  • Chronic obstructive pulmonary disease (COPD)
    COPD is a group of lung diseases that cause you to have trouble breathing.
  • Congestive heart failure (CHF)
    CHF is a disease that occurs when your heart is too weak to pump blood.
  • Coronary artery disease (CAD)
    CAD happens when a substance called plaque builds up in the arteries that supply blood to the heart (called coronary arteries).
  • Diabetes (Sugar)
    Diabetes is also known as “sugar”. Diabetes type 1 is a disease where your body can no longer make insulin. Diabetes type 2 is a disease where your body cells are less sensitive to your insulin. Both types can cause high blood sugar levels. MHS Diabetes Education Program (PDF)

Specialized Health Programs

The following programs are also offered through Case Management. 

  • Attention-Deficit / Hyperactivity Disorder (ADHD)
    Children with ADHD have trouble paying attention, controlling impulsive behaviors (may act without thinking about what the result will be), and in some cases, are overly active.
  • Bipolar Disorder
    Bipolar disorder, also known as manic-depressive illness, is a brain disorder that causes unusual shifts in mood, energy, activity levels, and the ability to carry out day-to-day tasks.
  • Chronic kidney failure (CKD)
    CKD is a disease where the kidneys become less able to clean waste and extra fluid out of the blood.
  • Depression
    Depression is a mental illness that affects your thoughts and feelings. It can cause problems in your everyday life. It can make you feel sad or down for no reason. People who have family members with depression are more likely to have it too.
  • Hepatitis C
    Hepatitis C is a virus that attacks the liver. Many people show no signs of an infection, but those who do may experience fatigue, nausea, or a lack of appetite.
  • HIV
    HIV is an acronym for the human immunodeficiency virus. This virus can be spread through unprotected sexual contact, blood transfusions, or sharing contaminated needles.
  • Stop Smoking
    MHS encourages you to quit using tobacco products or smoking today. Tobacco smoke contains nicotine, a drug that is addictive and can make it very hard, but not impossible, to quit. More than 400,000 deaths in the U.S. each year are from the use of tobacco products.

*This card may not be used to buy alcohol, tobacco or firearms products. This card is issued by The Bancorp Bank pursuant to a license from Visa U.S.A. Inc. The Bancorp Bank; Member FDIC.
