Hoosier Care Connect
Hoosier Care Connect (HCC) is an affordable health care insurance plan through Indiana Medicaid. It is for individuals aged 59 years and younger, blind or disabled and who are also not eligible for Medicare. Hoosier Care Connect members will receive all Indiana Medicaid-covered benefits in addition to care coordination services. Care coordination services will be individualized based on a member’s assessed level of need determined through a health needs screening.
Who Qualifies for Hoosier Care Connect?
Individuals who meet these eligibility factors can apply:
- Individuals aged 59 years or younger
- Blind individuals
- Disabled individuals
- Individuals receiving Supplemental Security Income (SSI)
- Individuals enrolled through M.E.D. Works
- Some foster care children
Members of the Hoosier Care Connect plan will select a managed care entity (MCE) responsible for coordinating care in partnership with their medical provider(s).
All services must be medically necessary. Your Primary Medical Provider will work with you to make sure you get the services you need. These services must be given by your Primary Medical Provider or by another provider that your Primary Medical Provider refers you to.
Some services may:
- Have coverage limits.
- Need a doctor’s order.
- Need prior approval.
*Some Medicaid members may not have all the benefits listed.
How to Become a Hoosier Care Connect MHS Member
With HCC, you have a choice of health plans to help coordinate your care. Members select a health plan or will be assigned to one and will stay with that health plan through the calendar year.
