Provider Incentive Programs

Managed Health Services (MHS) administers provider incentive programs to support quality improvement, improve health outcomes, and promote provider performance across Indiana Medicaid populations. Programs align financial incentives with evidence-based care and measurable quality outcomes, including preventive services, chronic disease management, and care gap closure. 

Program Descriptions

Provides incentive payments to our contracted Primary Care Providers based on performance on defined preventive, screening, and chronic care measures using claims-based, HEDIS-aligned metrics. Incentives are earned through the closure of attributed member care gaps. Program data and eligible members are accessible through MHS Secure Provider Portal.  For additional information, providers can refer to their contract or contact their Provider Engagement Account Manager. 

Provides incentives for activities that support care coordination and ongoing management of member health conditions. Program data and eligible members are accessible through MHS Secure Provider Portal.  For additional information, providers can contact their Provider Engagement Account Manager. 

Provides enhanced reimbursement of $50.00 for initial tobacco cessation counseling services to support reduction of modifiable health risks.  For additional information, providers can contact their Provider Engagement Account Manager.
