We believe you are the most important person when it comes to your health. You have the right to make choices about your care. We want you to be part of every decision.

Sometimes, people get very sick and cannot tell the doctor what they want. That is why there is something called an Advance Directive. It is a form that lets you write down what kind of care you want. You can also choose someone you trust to speak for you if you are unable to.

After you fill out the form, ask your doctor to keep it in your file. You can also talk to your doctor about it. This helps make sure your wishes are followed. You can change your Advance Directive anytime. Just let your doctor and family know. This way, even if you cannot speak, people will still know how to care for you the way you want.
