Step 5: Make an appointment

Now that you have a provider, it is time to make an appointment.

Call your doctor’s office. Make sure you have your insurance card handy and know what you want.

You may need to have an initial appointment before the provider will see you for a sick visit.

When you call, you should say your name and if you are a new patient. You should say why you want to see the doctor. You also should be prepared to give your insurance and other information.

You should ask if there are any forms you should fill out before going to the office. You also should ask if you should bring anything to your appointment. And ask what to do if you need to change or cancel your visit.
