Columbus Regional Health - 2023 Summit Award

For reporting year 2022, Columbus Regional Health, was recognized for their efforts in providing quality health care to MHS members. Overall, 40% of all care gaps, were closed for members enrolled in Hoosier Healthwise, Hoosier Care Connect and the Healthy Indiana plan. Care gaps closed included, well child, prenatal and post-partum care, screenings, immunizations, and preventative and ambulatory health services.

Additionally, Columbus Regional Health, is a participant in the Continuity of Care Risk Adjustment program. CRH ensured that members identified for this program were seen for chronic disease management programs for asthma, heart, lung, and cancer conditions. CRH submitted almost 70% of their agendas for the 2022 reporting year.
