Important: Redetermination and Eligibility Checks
Indiana Medicaid members must have their eligibility renewed every 12 months. This is called renewal or redetermination. It includes checks of your income to make sure you still qualify. MHS wants to make sure you have the info and support you need. There are steps you can take today to make sure you are prepared.
Go to your FSSA Benefits Portal
Here, you can find the Indiana Medicaid renewal form and other info about your benefits. Create an account if you don’t have one. On the benefits portal you can:
- Submit your application
- Review your benefits
- Print proof of eligibility
- Report changes in family size, income, or address
- View your notices
Read your mail from FSSA.
Watch your mail. If you get a form from FSSA about your health insurance, here is what you need to do:
- Fix Mistakes - If the information on your form is wrong or missing, write the correct info on the form.
- Review, Sign and Send Required Documents - any missing information or incomplete forms can lead to your paperwork being rejected.

- In person: Visit your local Division of Family Resources (DFR) office
- By mail or fax: Instructions will be on your notice

You may receive a letter in the mail, email, and/or phone call from MHS to let you know when to redetermine. Make sure you let MHS and FSSA know if you have a change in address, email, or phone number.
