Zip Code + 4 Reminder

MHS would like to remind all providers billing with a National Provider Identifier (NPI) that all claims require the zip code + 4 format for the billing provider addresses. This information is necessary to ensure a one-to-one match for provider identification.

In cases where a one-to-one match cannot be established using the zip code + 4, please ensure you include the taxonomy code that was submitted during your Indiana Health Coverage Programs (IHCP) enrollment. Including the full zip code + 4 and the taxonomy code, when applicable, ensures proper identification of the claims. If MHS cannot make a one-to-one match to identify the specific NPI ID associated with a claim, the claim will be rejected or denied.
