New Claim Form Required; Handwritten Submissions No Longer Accepted

Managed Health Services (MHS) is changing our policy as it relates to corrected claim and some COB claim submissions. Effective October 1, 2014, MHS will only accept the new version of the CMS-1500 form dated 2/12. Additionally, effective December 1, 2014 both CMS-1500 and UB-04 should be free of handwritten verbiage. This includes corrected claims and coordination of benefit claims submitted under the “90 day provision.”

CLIA Services Claim Submission Reminders

Providers billing an initial claim, resubmission, or corrected claim for CLIA services to MHS on or after January 6, 2014 must include a valid and appropriate CLIA number (i.e., either a CLIA Certification Number or CLIA Certificate of Waiver number, as applicable) with the claim. If a required CLIA number is not included on the claim, one of the following rejects will be applied:

New Date Change for including National Provider Identifier of the Ordering, Prescribing, or Referring Practitioner on Medicaid Claims

Effective March 1, 2014, MHS will require providers to include the National Provider Identifier (NPI) of the ordering, prescribing, or referring practitioner (OPR) on Medicaid claims as outlined in Indiana Health Coverage Programs (IHCP) bulletin BT201233.

Claim form Changes

Beginning January 6, 2014, MHS will process professional paper claims submitted on the revised CMS-1500 claim form (version 02/12). The CMS-1500 (02/12) form is available from the National Uniform Claim Committee (NUCC) website at nucc.org or your current CMS 1500 forms provider. The form has been revised with changes to more adequately support the use of the ICD-10 diagnosis code set.
