channelMHS and Cenpatico are working together to bridge the gap that often occurs between physical and behavioral healthcare providers who share a patient — by sharing that patient’s health information.
MHS encourages the use of the Behavioral/Physical Health Coordination form, which can be found on the Provider Forms and Member Forms pages. Providers can then easily, efficiently and legally exchange important patient health information. The goal is to enhance providers’ ability to wholly understand their patients’ health status and needs.
