MHS’ disease management program focuses on members with specific chronic diseases, and was developed to assist patients’ understanding of their disease and empower them with self-care strategies.
Our disease management program is designed to reinforce your treatment plan. We will provide you with updates and any test results as well as information, as appropriate, regarding member compliance with testing, medications and self-management. In addition, our staff may contact you to share clinical information or to verify services received.
MHS will help ensure that your patients understand their conditions and how to manage them by providing…
- Support from our nurses, social workers, and assisting staff;
- Educational materials;
- Information on wellness and self-management classes;
- Support of home health to provide care and/or additional education in the members’ homes, as needed;
- And information on MHS community events and local support groups.
Health Condition Management
The following Health Condition Management programs are available to MHS members:

- Preinatal Depression

- CKD
*Depression management services are delivered based on referral from existing clinical management programs relative to member need and program eligibility.
MHS Disease Management programs are available to members with specific conditions who will benefit from long-term education and self-management support as well as monitoring of their health status. Through our programs, individuals learn self-management skills to improve their health or the health of their children.
The programs are designed to deliver member education, facilitate timely, high-quality outcomes for its members, and promote appropriate use of healthcare resources. Care management staff focuses efforts on providing the latest evidence-based educational information to the member, while addressing the areas of medication compliance and symptom management.
MHS’ Disease Management goals are to…
- promote overall health;
- prevent exacerbation of symptoms;
- increase member program participation and touch rates
and enhance member satisfaction through education and targeted interventions
MHS members are automatically invited to enroll in our programs after they are identified with a qualifying condition. Membership in a Disease Management program is voluntary and members have the option to end participation at any time.
