Who Qualifies?
The MHS Indiana Check-Up Plan is an individual health insurance plan. It is for people who are eligible for Healthy Indiana Plan offered by the State of Indiana, but are not able to join because the enrollment limit has been reached or there are no more funds.

How do I apply?
- Apply to the Healthy Indiana Plan. You MUST BE DENIED COVERAGE and receive a rejection letter from FSSA.
- Read all MHS Indiana Check-Up Plan information to decide if you want to apply.
- Choose a Primary Medical Provider (PMP) from the MHS Find a Doctor search.
- Download, print, and fill out the Indiana Check-Up Plan application (PDF). Attach a copy of your rejection letter to your application. Mail the application and letter to our Indianapolis office:
